Basis. Combined V-belt drive and pump-bearing failure on the RM2 Pump 2 set, 20 July 2026 at approximately 03:30, recorded in failure report OPT-RM2-VFR-20260720 (final issue, validation lock applied). The set is out of service under repair and the client-confirmed service-state update of 14 August 2026 records the sensors physically removed. Closes out on an authorised return-to-service commissioning record, followed by re-baselining of the drive and pump measurements.

Provenance. Cells are the public release documents in optimaltrend-api/data/ projected by /client-condition-dashboard/condition-trend, which collapses correction chains through supersedesReleaseId and SHA-256 hashes each payload. Zones and labels are read from chartEvidencePolicy.findings and statusLabel. The RM2 Pump 2 set (P2, PM2) is shown for continuity but excluded from the action counts: the failure is recorded in OPT-RM2-VFR-20260720 and the set is out of service under repair. History spans 9 July to 18 August 2026 as overlapping daily rolling windows, so month and quarter roll-ups are not yet populated. Findings are advisory and require engineering review before any maintenance action is authorised.
